Add trainee name to top of mini pat form?

At the moment when filling in a min-PAT form, the page is not labelled with the trainee's name (obviously it is displayed on the link to access the form, and on the successful completion page). Not sure about other WPBA forms. It would be good to have this to minimise risk of confusion if distracted part way through completing.

Many thanks for getting in touch.
When you complete a mini-PAT on a trainee, there is a section at the top of the form which contains the name of the trainee being assessed. Other WPBAs have this information too. I've included a screenshot of a mini-PAT with the area highlighted so you can see what I mean.
