Step 1: Click on the '+ New Review' button in the trainee's portfolio.
The '+New Review' button can be found in a trainee's portfolio. Deanery administrators can find a trainee's portfolio by searching for the trainee in the search box.
Step 2: Select either 'Form R Part A' or 'Form R Part B' from the drop-down box.
Step 3: Enter the date of the review.
Step 4: Click on 'Start Review'
Step 5: Complete any available sections on the form.
The greyed out sections on the form must be completed by someone else.
Step 6: Click on 'Submit' or 'Save Changes'.
If you 'save changes' the form will remain in your account as a draft and will not be visible to others. If you 'submit' the form it will be available to those with the right permissions to view and complete it.
Both Part A & Part B of Form R must be submitted by the deanery and the trainee for the to be complete and be recorded in the trainee's portfolio as activities.
