Trainees & deaneries in the northeast can now complete a Form R from their respective Portfolio Online accounts.
Step 1: Click on the '+ New Review' button.
The '+New Review' button is located on a trainee's portfolio card.
Trainees can find their portfolio card on the dashboard of their account. Deanery Administrators & Heads of School can find the portfolio cards of all trainees in their school on the 'School Page' under the section 'Trainee Portfolios'.
Step 2: Select either 'Form R Part A' or 'Form R Part B' from the drop-down box.
Form R (Part A) asks for personal and contact information that is used to maintain accuracy of data.
Form R (Part B) asks for information about your recent scope of practice, time out of training, and declarations relating to revalidation.
Step 3: Enter the date of the review.
Step 4: Click on 'Start Review'
Step 5: Complete any available sections on the form.
The greyed out sections on the form should be completed by someone else.
Step 6: Click on 'Submit' or 'Save Changes'.
Saving the form will mean the form remains in your account as a draft and will not be visible to others. Once the form has been submitted it will be available to those with the right permissions to view or complete.
