The Psychiatric Supervisor Report was previously called a 'Level 1 Supervisor Report' and is an 'end-of-post' or 'mid-term' structured report which can be completed either by the educational or clinical supervisor and is typically done at the end of a placement.
The Psychiatric Supervisor Report is a multi-user form and is designed to be initiated by the trainee. The trainee should comment on each area of the curriculum prior to the review and submitting the form. Once the trainee's comments have been submitted, they are made visible to the supervisor completing the form.
