Complete a Mini-PAT Self Assessment

Trainees must complete a Mini-PAT self-assessment as part of the Mini-PAT process. The Mini-PAT self-form can only be completed once the Mini-PAT is in progress and must be completed before the Mini-PAT deadline. For more information on the Mini-PAT, see the article Undertaking a Mini-PAT.

1. In your portfolio on Portfolio Online, choose Portfolio > Assessments.
2. Click on +New Self Form on the mini-PAT and complete the form.
3. Click on Submit.

Once the form has been submitted you cannot make any more changes. If you click on 'Save Draft' the information you have entered remains private until you submit the form.
