How do I amend an ARCP Outcome Form that has been released to a trainee?

Once an ARCP Outcome Form is complete, the deanery releases it to the trainee.

If an ARCP outcome form needs amending after this stage, we need to take additional steps to ensure that the trainee and head of school are aware of any amendments to the ARCP Outcome Form.

If the change is to the outcome itself, the head of school must contact the Portfolio Online Support Team via email on [e-mail] to confirm that the trainee has been informed of the change and to authorise the Portfolio Online Support Team reopen the form.

If the change is to any other part of the form, the deanery can contact the Portfolio Online Support Team via email on [e-mail] to confirm that the trainee has been informed of any changes and to request the PortfolioOnline Support Team reopen the form.

Once any modifications have been made, the chair of the panel or post-graduate dean must sign off the ARCP Outcome Form again, a deanery administrator must then release the form to the trainee and the trainee can then acknowledge the ARCP outcome by signing it off the form once more.
